Appointment
We value your time. We make every effort to see you at the appointed time, and with as few visits as possible. In order for us to be efficient with the time that we have scheduled for you, we would ask that you also be prompt. All appointments are confirmed 48 hours prior to the actual date. Many times if not available by phone, we will leave a message to confirm your visit with us. We ask that you be courteous when you receive that confirmation call and please respond back to us promptly. This respects our time and saves unnecessary calls.
We value our patients and their dental health. Your appointment is an important part of maintaining your dental health. Keeping this in mind it is our policy that should you have to cancel your appointment we need at least 2 business days notice. Should you miss your appointment entirely and not give us pre-notification, we will charge you for the amount of time set aside for you.
For new patients, we ask that you bring a picture id, your insurance card, any x-rays that you may have from other offices, and a list of medications that you are currently taking. We ask that you arrive 15 minutes prior to your dental visit to fill out the necessary paperwork.

Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Our office policy is for you to pay your portion at the time of service. All insurance co-pay balances are to be paid at the time of service. In addition, for the uninsured, all services are to be paid in full at the time of the appointment unless prior written arrangements have been made with our office manager. We accept checks, cash, MasterCard, Visa, Discover, and Amex. We also offer a flexible payment plan called Care-Credit. This company offers “No Interest” financing or low monthly payment options so you can get the treatment you need at an affordable repayment rate.

With CareCredit you can finance 100% of your dental care and there are no upfront costs, no annual fees, and no pre-payment penalties. So, you can begin your treatment today and conveniently pay with low, monthly payments.
CareCredit offers a full range of payment plans so you can find one that works best for you. With the popular No Interest Payment Plans* there are no interest charges if you pay your balance in full within the specified time period. Monthly payments can be as low as 3% of your balance. For procedure fees from $1,000 and up, CareCredit offers 24, 36, or 48 month plan options with low monthly payments available.
CareCredit can be used by the whole family for ongoing treatment without having to reapply and by using dental financing such as CareCredit for your dental care, you can save your other credit cards for household or unplanned expenses.

Dental Insurance
We accept most insurance plans, with the exception of any DMO plans, and will gladly process your claim. Insurance companies generally cover only a portion of treatment costs. It is the policy of the office that your portion of the treatment costs not covered by insurance be paid at the time of service. In special cases and by pre-arrangement with our office manager, for large treatment cases special allowances can be made. Ultimately it is your responsibility to pay any balance not paid by your insurance company. Once your treatment costs are presented to you, our financial team will help you arrange payment options. We will prepare you ahead of each scheduled appointment as to what is expected for your co-payment due at the visit. For large treatment plans we will submit a preauthorization on your behalf to get the insurance’s written permission to do the procedure, which also gives us the exact amount that we will expect.
